Scheduling Process

At the Learn to See Vision Clinic, we take a personalized approach to patient care which starts even before your first visit. Many of our patients have complex visual symptoms or learning-related concerns, so we take a different approach than most practices when it comes to scheduling.

Step 1: Phone Consultation with Dr. Les
Before scheduling any appointments, Dr. Les speaks with each prospective patient (or their parent/guardian) over the phone. This allows him to understand your symptoms and your history, answer your questions, and tell you about the office.

Step 2: Complete Your Online History Forms
After speaking with Dr. Les, you’ll receive an email with a link to our secure online history forms. These forms must be fully completed and submitted before we can schedule your exam.

Important: Be sure to sign and hit SUBMIT at the bottom of the Providers/Policies tab, or your information will not be sent to us.

Need help with the forms?
If you're not comfortable using a computer, we recommend asking a family member or friend to assist you.

Step 3: Schedule Your Appointment
Once we receive your completed forms, we’ll send you a follow-up email with available dates and times. Let us know what works for you as soon as possible as these times are not reserved. If these times do not work, let us know your availability so we can continue to look for another options.

Step 4: Send Us Your Previous Eye Exam Records
​​​​​​​If you’ve had any previous eye exams, please have those records faxed to our office before your appointment. They provide valuable background and can help guide your care.
